Who can submit
Business employers and employment agents may use this form for MOM-issued pass holders. An employment agent must obtain the employer’s written authorisation.
One holder per submission
Submit a separate form for each pass holder. The public instructions require corporate login and state that the outcome will be emailed within four weeks.
Domestic worker cases
Employers of foreign domestic workers are directed to the separate domestic-worker refund form. Follow the evidence requirements displayed in the relevant official form.
